Care Roots was founded from personal experience and a deep understanding of what it's like to carry the weight of coordinating care for yourself and loved ones, balancing appointments, understanding treatments, and communicating across providers as a mother, wife, daughter, and clinician.

It was difficult to watch my mother manage all the intricate details to care for my grandparents across multiple providers and painstakingly difficult to watch my father try to navigate the healthcare system through cancer care. As a Care Management Nurse Practitioner, I've watched in agony as my patients move through a system that assumes they have support they simply don't have. Oh, the stories I could tell!

As an Adult/ Geriatric Certified Nurse Practitioner with over 15 years of diverse experience in adult/ geriatric care, including almost a decade in complex care management, I act as your dedicated clinical partner- coordinating care, advocating for your needs, and keeping every piece of your healthcare journey connected so you can focus on what matters most: health, peace of mind, and living life fully- on YOUR terms!
